[Approval] Why is the approver incorrect or the application automatically approved?

Scenario one:

When the approver and the submitter are also the department head, the application will be approved by himself/herself.

Scenario two:

The approver is set as the department head but the department head hasn’t been set up in [Department]. The system will regard the approver as null and the application will be approved automatically or by the Admin.
